Primary Care Services
At Fairfax Family Practice Centers, we provide continuing and comprehensive pediatric and adult health care, including most preventive, diagnostic, and therapeutic services for a wide array of acute and chronic problems. As family physicians, we are dedicated to treating the whole person. Family medicine’s cornerstone is an ongoing, personal patient-physician relationship focusing on integrated care. Unlike other specialties that are limited to a particular organ, disease, age, or sex, family medicine integrates care patient information for both genders across the full spectrum of ages within the context of community and advocates for the patient in an increasingly complex health care system.
Family medicine is a medical specialty devoted wholly to comprehensive health care for people of all ages. It is the primary division of health care that provides continuing and comprehensive care for the individual and family across all ages, genders, diseases, and parts of the body. Care is based on knowledge of the patient in the context of the family and the community, emphasizing disease prevention and health promotion. The aim of Family Medicine is to provide personal, comprehensive, and continuing care of the individual throughout every stage of life. Fairfax Family Practice Centers is especially devoted to making every patient’s needs and concerns a priority. We are a certified Patient Centered Medical Homeby the National Committee for Quality Assurance (NCQA). NCQA’s Patient-Centered Medical Home is an innovative program for improving primary care. The PCMH program encourages and facilitates partnerships between individual patients and personal physicians, and when appropriate, the patient’s family. FFPCS is organized as physician-led care team that provides for every patient’s health care needs and coordinates care across the health care system.
